Question:

For the pulmonary angio codes, if the RHC is done in the cath lab but the pulmonary angiogram is done in IR which codes would be used where?

Answer:

A right heart catheterization is captured with 93451. This code is used for catheterization of the right atrium and or right ventricle with pressure measurements, oxygen saturation, and measurement of cardiac output when performed. This is typically a cardiac cath lab service. If in a separate session in IR, a patient has a pulmonary angiogram, this would be captured with two sets of codes, one set for catheterization from 36013-36015, and a set for your angiographic service, captured by 75741 or 75743. If during a single cardiac cath lab visit, a patient has a right heart catheterization with the additional service of a pulmonary angiogram all in one encounter, this would be coded to 93451 for the right heart catherization, with the addition of an add on code from range 93456-93575.
