When do we report modifier 91?

Answer:

Modifier 91 should be used to indicate laboratory services performed on sequentially collected specimens when reported by the same individual physician or other qualified healthcare professional (QHP). It is inappropriate to use modifier 76 or 77 to indicate repeat laboratory services. Separate consideration for reimbursement will not be given to laboratory codes reported with modifiers 76 or 77. Understand that modifier 90 is reserved for use by Independent Laboratories.
