When a stop time has not been recorded for the infusion, can an IVP be coded?
The answer to this question varies among instructions from individual payers. The generally accepted rule is that sufficient information should be available within the medical record documentation to either confirm or calculate the stop time. This means that either the actual clock time from first drip to last drip is accurately recorded or that time, volume infused, and rate of infusion are documented to allow calculation of run time. Calculated time must be based on information recorded in the nursing notes and not based on expected duration time documented within Medical Administration Record (MAR) information generated by the pharmacy for volume dispensed and recommended infusion rate.
